New vs. Old Tattoos in Laser Tattoo Removal

We are frequently asked, does it matter if my tattoo is old or new when trying to remove it?   There is a lot of conflicting information online.   So let’s set the record straight!

The truth: With the newer FDA approved Q-Switched lasers such as the Trinity laser system used at LeBeau Clinic in Pensacola,  the playing field has been leveled between older and newer tattoos.  Older tattoos have a slight advantage.  The ink content could be a simpler compound and is usually black in color.
Why? We have seen with time, aging, and sun exposure most tattoo inks will fade slightly. The lower the quality of the tattoo ink the quicker the ink breaks down.   Your body sees ink as a foreign invader or an unnatural element.  As a result,  your body’s immune system will try to remove some of the ink over time naturally.
So what is considered an old tattoo? In our opinion, tree years or more can be considered “old”.  No matter what the age of your tattoo – a number of factors determine how quickly a tattoo fades away including density of ink, how deep the tattoo artist went into the dermis, health of the client, location of the body (further from the heart, it takes a bit longer), immune system of the client, and physically activity. A marathon runner’s tattoo will fade more quickly than a couch potato.
How soon can I start a laser tattoo removal after getting a tattoo? Your tattoo should have time to heal before starting laser removal. Many people call us within the same day as getting a tattoo – in this case we will have to wait a little while to begin treatment, but we can go ahead and schedule your consultation at that point.
trinity rgb webThe Gold Standard of Tattoo Removal: The gold standard for tattoo removal remains treatment by an experienced provider using multiple wavelength Q-switched lasers in a series of treatments spaced 6-8 weeks apart.  The number of laser treatments will be based on the tattoo location, quality and character of the ink as well as ink density, patient skin type and overall health of the client.  This is a proven effective method for tattoo removal which works very well with minimal discomfort and very happy patients.  It is important to remember that tattoos are meant to be permanent and all tattoos present a significant challenge for removal.  Performing laser tattoo removal is an evolving skill which requires use of the appropriate laser wavelength, adjustment of the timing and power settings to match the appropriate tattoo inks, and requires significant experience and judgement on the part of the practitioner.

A quick search on the internet reveals a multitude of do it yourself tattoo removal options:  creams, gels, multi-step skin systems, etc.  A cursory review of the anatomy of skin is important to be able to understand why these options will not be effective at removing your tattoo.  The human skin has a top layer called the epidermis which is continually regenerating and sloughing the outermost skin cells.  This layer naturally provides an excellent barrier to any fluids, gels or creams applied to the skin due to the tightly packed and flattened cells.  Tattoo pigments are typically placed in layers within the deeper aspect of the skin called the dermis.  This layer does not chronically change or have cell turnover as the overlying epidermis does, thus, allowing the tattoo pigments to remain relatively stable within this part of the skin.

The basis of all of the marketed DIY tattoo removal techniques rely on topical skin application of “proprietary” formulations in an attempt to remove tattoo ink within the protected dermal skin layer.  Your tattoo artist did not simply apply the ink to your skin to reach the dermal layer and likewise, applying one of these topical formulations will not allow for ink to be removed from this deeper skin layer.
Below are some of the most heavily marketed topical DIY options:
QuickFade Tattoo Fading Gel – Topical gel containing TCA (Trichloracetic Acid) which they claim will allow for fading and eventual removal of tattoo ink.  This does not happen.  The company falsely claims that laser tattoo removal is “extremely painful” and leaves “unappealing scars” both of which are untrue.  QuickFade guarantees their product for 90 days but states that it requires 3-6 months to see improvement in tattoo.  6 month supply $179.99.
WreckingBalm Tattoo Removal and Fade System – a 3-step system which claims to remove the top layer of skin which then allows for “serum” to “break up the cellular matrix where ink is embedded”.  They have a nice, interactive website which I am certain sells a great deal of WreckingBalm but a quick look at Amazon reviews reveals what consumers really think of this product. “Complete scam”, “Snake Oil”, “Waste of Money”, etc. 10 month supply $   , Amazon review – 2 stars
Tat B Gone – Claims to be #1 topical product for effectiveness while remaining “gentle on the skin”.  Website states there is no TCA, no Chromabright, no Hydroquinone.  This is a system of creams which theoretically “permeate the dermis to help fade the tattoo away by attacking the ink”.  10 month supply $850, Amazon reviews – 1 star
Evanesco is a one step cream using “all natural” Anacardic acid from cashew nut oil.  This company actually claims to be able to remove your tattoo “within weeks” through a “dermatologic reaction” involving scabbing and scratching which removes the “dermis layer of your skin”.
Removing the dermis layer of the skin is the equivalent of a 3rd degree burn and will  result in a significant scar 100% of the time.  It is very unfortunate that this is able to be peddled on the internet to unwary consumers. 1mL box of Evanesco is $79.95, No available reviews.
InkBusters, TAT-Med, Profade, Tattoo-Off, Dermasal, Rx-tattoo are all similar variations of topical creams, ointments and gels claiming to be able to remove tattoos, all of which are ineffective.  Unfortunately, consumers desperate to have their tattoos removed often fall prey to the unscrupulous marketing efforts and ineffective products sold by these companies.

Myth #1 : One Type of Laser Can Remove all Tattoos
Do you believe the myth that there is one laser that is “best” for tattoo removal? This is false. We find that multiple laser wavelengths are needed for a safe and complete removal. Usually taking 2 or more lasers. Don’t believe the hype—from anyone—who says they have one laser that can remove all tattoo colors.
At LeBeau Clinic, we have the most advanced, state of the art laser for tattoo removal which offers full color removal. LeBeau Clinic uses the Astanza Duality and Eternity Lasers, which is a combination of a q-switched Nd:YAG (1064nm & 532nm) and a Ruby (694nm) laser, creating 3 wavelengths that makes it able to treat all colors of tattoos effectively.  These wavelengths are as follows:
1064nm     blacks, browns, and some purples
532nm     reds, oranges, some purples, and helps to fade yellows

Myth #2 : Tattoo Removal is More Painful Than Getting a Tattoo
I am sure everyone has heard this statement. This myth is what keeps people from trying the procedure. For smaller tattoos, a local anesthetic can used to numb the site (eyebrows & ring finger tattoos, etc). Larger to medium tattoos can have a topical numbing agent applied before treatment. Also, a dermal cooler, a device that keeps the skin cool during the treatment really helps out. At LeBeau Clinic we use the Zimmer Cryo which blows freezing cold air on the skin to numb it before and during laser treatments.  If you are having a lot of pain during a laser treatment, let your health care professional know. They can take safe measures to help you lessen the pain.
Myth #3 : Tattoo Removal Causes Scarring
It is a common fear and belief that laser tattoo removal will scar you for life. As with any medical procedure, there is risk and benefit. On average, the probability of scarring is around 5% from laser treatment. That figure can be decreased by the client being very careful to follow the aftercare instructions from the health-care provider. Not all clients are suitable for laser treatment. Risks for tanned skin, darker skin tone, history of keloiding, pregnancy, Accutane medication, immune system disorders, some body locations, and other medications and disease processes.
After your tattoo removal treatment at LeBeau Clinic, we will cover the area and go over detailed aftercare instructions. We will ask the client to keep the area covered for 3 days post treatment, changing the bandage 2-3 times a day. Some may experience side effects such as redness, tenderness, swelling, bruising, blisters, and scabs. These are normal and should be left alone as much as possible to now cause texture change or scarring of the tattoo. Clients should cover the area for 3 days using Neosporin, Aquaphor, or similar.
Some clients with darker skin tones (skin types III-VI on Fitzpatrick scale) may have slight skin color change as part of the tattoo removal called hypopigmentation (skin gets lighter) or hyperpigmentation (skin gets darker) in the area of the tattoo. In the great majority of cases, this skin tone change is transient and can be alleviated with sun exposure or skin lighteners if client desires. Time is the ultimate helper here.
Myth #4 : Tattoo Removal Creams vs. Laser Removal
So many people want to know if “removal creams” found on the internet will take off their tattoos. The short answer is “no.” When you get a tattoo,  ink is placed deeply in the skin.  Rubbing a cream on top will not achieve removal of a tattoo. If this method worked there would be no need for lasers. Nor would the laser manufacturers spend millions of dollars for research, development, and marketing if a simple magic cream could do the job.  If there were a product like this that removed tattoos easily,  it would be on every shelf in every drug store in America.  The cliché is true—if it sounds to good to be true it probably is.

How much would the removal of a tattoo or a fading for a cover up cost?

The cost of tattoo removal is based on the size of the tattoo and the number of treatments required for clearance.  We use a formula based on the location of the tattoo, the color density, layering, scarring, and skin type to obtain an estimate for the number of treatments likely to be required.  The cost per treatment is based on the size of the tattoo.  A visit to a knowledgeable and experienced provider should be able to give you a good idea of the anticipated cost for removal.

New tattoo or old tattoo: Which one responds to laser removal better?
Older tattoos typically require fewer treatments for clearance compared to newer tattoos. This can be the result of the ink quality as well as the presence of an already present immune response in the area of the tattoo for older tattoos which facilitates clearance of the ink particles following treatment.  The Picosure is a single wavelength laser, Alexandrite 755nm, and provides a shorter pulse duration but less overall energy than other systems and due to the single wavelength is not optimal for treating tattoos with certain ink colors (primarily reds).  Systems such as the Astanza Trinity laser provide three wavelengths (1064nm, 532nm, 694nm) with adequate energy to treat tattoos of all colors.
Tattoo removal under $1,000?
The overall cost of your tattoo removal will depend on several factors other just the size and ink colors.  At my clinic, we use a formula based on patient skin type, ink density and placement, tattoo size and location and presence of pre existing scarring or an underlying tattoo.  This formula allows us to accurately estimate the number of treatments as well as the cost needed for clearance of the tattoo ink.  You will likely need between 6-15 treatments by an experienced provider using an appropriate laser to obtain adequate clearance.  No ethical provider will give you a guarantee that you will be able to obtain the result you want for under $1,000 as there are sometimes variables which cannot be predicted.

Will I experience redness after a tattoo removal session?
Redness (erythema) following laser tattoo removal is not uncommon. I would expect that you see continued resolution of the erythema without any intervention. As always, I recommend you return to your provider for an examination of the site and further reassurance.
How many laser sessions will it take to make my tattoo light enough to cover up with make up?
You should expect good results if seen by an experienced provider using a quality Q-switched laser. You may begin treatments as soon as the site is completely healed but an accurate estimate of the number of treatment sessions required can only be given after an examination and further history obtained by your provider.
How many sessions will I need to remove a small black tattoo?
Once your tattoo is evaluated and your provider has elicited the appropriate history of the tattoo, he or she should be able to give you an estimate of the number of treatments required for clearance of the tattoo ink. Variables such as the density and quality of the ink, layering and scarring in the area are questions that will need to be further elucidated before an accurate estimate could be given.
I regret my tattoo. What can I do?
You are not alone as tattoo regret is actually quite common.  Luckily, your tattoo should be able to be cleared very successfully.  I recommend you find an experienced provider in your area who uses a quality Q-switched laser to have a consultation for laser tattoo removal.

Dr. LeBeau uses state of the art lasers to direct energy at the ink in your tattoo for a fraction of a second. 2. Can all the colors be removed from my tattoo?

At LeBeau Clinic, we can remove most colors completely. While some colors require more treatments than other colors, even the most challenging colors will usually fade to a point where they’re nearly impossible to see.
3. Will my tattoo be completely gone after my first treatment?
Most tattoos require multiple treatments for full removal.
4. Will I see a change in my tattoo after each treatment?

Because each person and every tattoo is unique, everyone responds to laser tattoo removal treatments differently. Some people notice a distinct change with every treatment, while other people see changes after several treatments. When you follow our clinical staff’s advice, you receive the best results as soon as possible.
5. Are there limitations after laser tattoo removal treatments?
After treatment you can resume most or your usual activities as long as the treated area is not scratched or rubbed. You should keep the treated area out of the sun and covered while in the sun until your skin has healed.
6.Can I have just remove a part of my tattoo?
Yes! Our technology are extremely accurate. Quite often we remove just part of a tattoo so that you can have it redone correctly or look they way you want.
7. Will I have a scar after treatment?

When you follow our aftercare instructions, the chances of scarring are nearly nonexistent. If you have any preexisting scarring, those scars will remain as our lasers only remove ink, not scars.
8. Does tattoo removal hurt?

Pain is very subjective to each person. Your comfort level is greatly improved when we use our specially designed device to blow cold air on the are being treated to numb your skin.

Patients are seen 3 months post treatment.  Both were concerned about a dark spot on their face and received one full face BroadBand Light treatment and ProFractional Resurfacing on the dark spot that they were concerned about. This patient is a 41 year old Caucasian female who had significant photoaging secondary to excessive sun exposure. The results were obtained from a single treatment with Broad Band Light (BBL) and a Sciton MicroLaser Peel. Patient is seen one month after treatment at which time there is significant improvement in the skin texture, pigmentation and fine lines and wrinkles. The combination of BroadBand Light and MicroLaserPeel provided excellent improvement in abnormal facial dark spots as well as broken capillaries. In her follow up pictures below, you can see an improvement in overall texture and quality of the facial skin giving a more youthful appearance.
